1. Goto a merchant

3. Open character menu by pressing 'C' then click on your scouter -
As your forum points increases you get new title.

No its not a bug that's how the game works. If you die a lot the durability of your armor goes down (Hammer icon lower right) and if it reaches 0 then it breaks. If the scouter breaks then you will see mobs as ???. Visit a merchant and repair it.

Wear or repair your scouter.

Logging off when transformed crashes the game. I tried with pure majin not sure about others.
