Is there a reason why during the first mission (Where you gotta kill Radditz) that Piccolo isn't missing his arm and Goku never gets a hole blown through him? I always thought that was weird.

They wouldn't have to make it "m", as it was done in the first Budokai in 2001.
However, mid/late in the PS2 era, the laws in Japan for gore in video games changed. Things like excess blood, decapitation and missing body parts, are censored in games there now.
This has been a thing since Resident Evil 4 at least. In most areas, the chainsaw guy takes your head off and you see it. In Japan (where it was made), they zoomed in to the torso and never showed the headless body.
