revive bug

hello for those that are reading this i encountered a glitch or bug in where i revived for the first time and the enemy name have changed to ???
i haven't been able to change this by reviving again or by, logging in again. this problem still persist for me and its hard to do my quest when enemy models look similar but have different names. i have also noticed that the enemies health bar and energy bar also have ??? instead of the numbers to show how low it is .
ps. only for enemy models now friendly npc, if there is any help that i can get it would be greatly appreciated
